Stop it, undo it, prove it: the examiner's test for AI agents has arrived

State bank examiners now have a work program that asks, for any agentic system, where a human intervenes, what is logged, whether actions can be reversed and whether the system can be restricted or halted CITED Source C02. A banker survey cited alongside it found 72% feel least prepared for kill-switch protocols or reporting AI failures to regulators CITED Source C02. The same three questions (can you stop it, can you undo it, can you prove what it did) now run through payments, prior authorization, factory robots and grid operations. This edition tests four fresh deployments against them.

  • Supervisors are writing the agent questions down. The Conference of State Bank Supervisors (CSBS) released an AI supervisory framework on September 16 that explicitly covers generative and agentic AI; adoption is at each state's discretion and it creates no new legal obligations VERIFIED Source C01. Federal Reserve Governor Christopher Waller told Sibos on September 29 that fraud systems "calibrated to human behavior, may not translate well to agents" VERIFIED Source C03.
  • Healthcare's test is evidence, not capability. FDA's discussion paper on generative AI devices, open for comment until Monday, October 19, proposes a competency-assessment approach to premarket review and touches agentic AI only briefly VERIFIED Source C16. 2. The Thursday thesis: stop, undo, prove

Problem. Most agent governance in regulated firms was designed for models that recommend. Agents act: they move money, book appointments, file notes, load test rigs and flag grid outages. When something acts, a supervisor asks different questions. The CSBS work program, as reported by American Banker, asks how a bank defines the actions a system may take, where humans intervene, what is logged, whether actions are reversible and whether the system can be restricted or halted CITED Source C02. Those are operating questions, not model questions.

Cause and effect. Firms invested first in model evaluation and policy. The weak point is the run-time layer: 72% of surveyed bankers say kill switches or AI-failure reporting is where they are least prepared CITED Source C02. Waller added a second gap: authentication must now prove an agent's authority to pay, and higher business-to-business values "amplify the financial exposure from agent errors or unauthorized actions" VERIFIED Source C03. If you cannot halt an agent, reverse its last action and show the log, every new use case adds risk faster than value.

Our read. Classify every agent by how reversible its actions are, then set the controls to match. A draft a person signs is low risk even when the model is weak. A payment that settles in seconds is high risk even when the model is strong. We call this the AEGIS Reversibility Ladder (Section 10) PROPRIETARY Source C04. AEGIS, the Agentic Enterprise Governance and Intelligence Standard, is the openly shared framework we use with clients.

The three questions, in plain words

  1. Stop it. Who can halt this agent, in how many minutes, and has anyone tried? (AEGIS Pillar 4, Controls and Human Oversight)
  2. Undo it. Which of its actions can be reversed, inside what window, and who pays when one cannot? (Pillars 3 and 4)
  3. Prove it. Can you show a supervisor what the agent saw, did and was allowed to do, from logs your firm holds? (Pillar 6, Monitoring and Response)

3. Frontier ledger: equal weight, what moved

Each group gets the same treatment: what happened, how sure we are, and why a regulated buyer should care. Select a tab. Where we found nothing verifiable in the window, we say so. Partner relationships do not affect placement PROPRIETARY Source C04.

xAI / SpaceXAI / Cursor

  • Rename, announced. On Sunday, October 4, Musk replied "Yes, we will make that change" to a suggestion to rename SpaceXAI as SpaceXSI; no timeline was given CITED Source C36.
  • Grok Bot as a model router. On October 7, Musk said Grok Bot will route each query to the best model, naming third-party models including Anthropic's Claude Opus 5.5; no company release beyond his post FLAG Source C37.
  • Cursor remote control. On October 6, Cursor shipped remote control of local desktop agents from its iOS app; it is on by default except for Enterprise organizations, where admins must opt in CITED Source C38.
  • Why it matters. Remote control of coding agents from a phone is an "off switch in your pocket" and a new access path. Enterprise default-off is the right posture; security teams should decide before users do.

4. Physical AI and robotics: what the data says

The International Federation of Robotics (IFR) reported on September 24 that more than 600,000 industrial robots were installed in 2025, up 11%, and that the operating stock reached a record of about 5.08 million VERIFIED Source C20. IFR forecasts 9% growth in 2026 (forecast) VERIFIED Source C20. The United States installed about 38,500 robots, up 12%, passing Japan for second place, while Germany fell 8% CITED Source C20.

  • Autonomy is shorter than the marketing. IROS 2026 in Pittsburgh (September 27 to October 1) drew a record 4,348 submissions; industry guests said 24-hour continuous humanoid or mobile-robot operation is not yet achieved, and one tracked an "autonomous operation ratio" on a 100-robot fleet running eight hours a day CITED Source C21.
  • Generalization progress, at home. Figure reported on September 17 that its Helix 2.5 model completed full tasks in 56% of trials across 30 unseen homes, up from 9% without pretraining (company-reported, homes not factories) CITED Source C24.
  • Contracts are long-dated. UK firm Humanoid's agreement with Schaeffler (May 13) targets "four-digit" numbers of wheeled humanoids by 2032, with a first box-handling phase from December 2026 to June 2027 (target) VERIFIED Source C23.
  • Every new task still costs engineers. AgiBot says its robots cannot learn a task from one demonstration; each needs pre-training and post-training by its engineers CITED Source C22.
  • Tariffs are a cost line, not yet a robot rule. No standalone Section 232 robotics tariff is in force; imported machinery falls under metals-derivative duties at a reduced rate through end-2027, and the robotics probe opened in September 2025 has no reported outcome CITED Source C25.

6. Four regulated case studies and their architecture

We chose one named deployment per sector, avoiding the names we used most in recent editions, and tested each against stop, undo and prove. We map each to the six governed layers of the AEGIS Reference Architecture Studio (875 L3 capabilities and 1,072 workflows across healthcare and financial services) PROPRIETARY Source C04. Results are company-reported unless marked. Where a layer is not disclosed, we say "typical pattern, not disclosed."

Financial services: Deutsche Bank's Source-of-Wealth agent

What happened. Deutsche Bank Private Bank said on September 22 that an agentic AI tool for Source-of-Wealth checks went live in its Singapore and Hong Kong booking centers at the start of September; it researches, documents and prepares Source-of-Wealth data, flags gaps and inconsistencies, and prepares assessments "for human review" VERIFIED Source C05. The bank's Emerging Markets region is "on track" to onboard about 30% more clients in 2026 than in 2025, a forecast for that region rather than a measured result of the tool VERIFIED Source C05.

Architecture. Inputs are the client case, client documents and "approved external sources"; the agent runs inside the bank's digital KYC platform; a person reviews and owns the assessment ("accountability remains with our people") VERIFIED Source C05. The model vendor, logging design and quality sampling are not disclosed (typical pattern: immutable case log, reviewer edit capture, sampled second-line review).

Stop, undo, prove. Stop: high, because the agent prepares rather than decides. Undo: high, because a draft is not a decision. Prove: depends on whether the case file stores the sources the agent used. This is Rung 1 on our ladder: draft for human sign-off PROPRIETARY Source C04.

Contrast. GoCardless completed what it calls the UK's first agentic account-to-account payment on September 22, setting up a recurring Direct Debit for the charity Trussell inside the FCA's AI Live Testing program VERIFIED Source C06. A payment mandate is a higher rung: it acts, and undoing it depends on scheme rules. American Express said on September 30 that it plans AI agents and accounts-payable workflows for corporate clients "in the coming months" (plan, not yet live) CITED Source C07.

Healthcare: payer agents that call providers

What happened. Becker's reported on October 2 that UnitedHealthcare's "Avery" agent calls in-network primary-care offices to book appointments for members and identifies itself as an AI on the call; Aetna also uses agentic scheduling VERIFIED Source C12. UnitedHealthcare targets more than 20 million members by end-2026 (target); no outcome data was published VERIFIED Source C12.

Architecture. Disclosed: a voice agent that places outbound calls on a member's behalf and discloses that it is an AI VERIFIED Source C12. Not disclosed (typical pattern): member consent capture, a provider directory and scheduling integration, call recording with transcript retention, and escalation to a human care advocate when a call goes off-script.

Stop, undo, prove. An appointment is reversible; a missed or wrong appointment for a high-acuity member is a harm. The same payer sector carries the counter-lesson: records from Medicare's WISeR prior-authorization model, obtained through an Electronic Frontier Foundation lawsuit, show a vendor warned CMS that a working product by launch was unrealistic, and Medicare Rights Center says the records show inappropriate denials VERIFIED Source C13. CMS says any non-affirmation is made by a licensed clinician VERIFIED Source C13. Health system leaders are still asking who carries liability when one agent fails to catch another's error CITED Source C14.

Manufacturing: AgiBot humanoids at Longcheer's final inspection

What happened (dated June 2026). AgiBot's Genie G2 humanoids picked tablets off conveyors and loaded them into final-inspection test equipment at contract manufacturer Longcheer in Nanchang during a six-day livestreamed trial (June 23 to 28); AgiBot reports 99.99% success across more than 17,000 tablets (company-reported) VERIFIED Source C22. The fleet ran on a secondary line from December 2025 before joining the main line in March 2026 VERIFIED Source C22.

Architecture. Disclosed: conveyor pick, insertion into test rigs, pass and fail sorting, multi-robot coordination on an 8 a.m. to 7 p.m. shift, skills pre-trained then post-trained by AgiBot engineers, and recovery from a communication fault in about two minutes VERIFIED Source C22. Typical pattern, not disclosed: a vision-language-action model on the robot, a fleet manager tied to the manufacturing execution system (MES), test results written to the quality system, and zoned safety under ISO 10218 with the humanoid standard ISO 25785-1 still in draft CITED Source C26.

Stop, undo, prove. Stop sits in the safety PLC and e-stops, outside the AI. Undo is physical: a mis-sorted unit can be retested, a dropped one cannot. Prove is the test rig's own record. The agent never owns the pass or fail decision; the test equipment does.

Energy: CAISO RC West's multi-agent outage analysis

What happened. CAISO's reliability coordinator for the western US uses OATI's "Genie" multi-agent system to analyze more than 500 next-day outage requests nightly in under eight minutes, versus about 40 analyst-hours per outage day before (vendor-reported, presented summer 2026) CITED Source C31.

Architecture. Disclosed by the vendor: an orchestrator with a workflow agent (nightly reports with anomaly flags), an advisor agent (answers over procedures and history, with citations) and a recon agent (watches critical outages and raises alerts); outputs appear in a chatbot, reports and a map inside the outage management system; hosting in private data centers aligned to NERC CIP; reviewers keep final authority with evidence shown beside each recommendation CITED Source C31. Typical pattern, not disclosed: read-only access to energy management and SCADA systems, role-based access and drift monitoring.

Stop, undo, prove. The design keeps agents on the advice side of the line: they cannot switch equipment. That is why it could be deployed in a NERC-regulated control room. The pressure is rising: about 3,800 MW of data-center load tripped offline in northern Virginia on July 22, and NERC issued a rare Level 3 alert on computational loads in May VERIFIED Source C30.

Insurance sits between finance and health and is the sector to watch: a Sapiens survey released October 7 found 25% of insurers rank as "Autonomous Leaders" and 53% of agentic AI use is still under human oversight (vendor-sponsored) CITED Source C09, while insurers are reported to be bracing for "rogue AI" claims under D&O policies FLAG Source C56.

Two more items with no date attached: NIST and CISA's final IR 8587 on protecting identity tokens (September 15) adds high-level considerations for AI agents CITED Source C57, and a European Systemic Risk Board committee is reported to have warned that banks relying on the same few models face "correlated behavior" risk; we could not locate the primary document FLAG Source C11.

9. Markets, labor and research signals

Labor

Gartner predicts more than 40% of agentic AI projects will be canceled by end-2027 (forecast, originally issued in 2025) CITED Source C59.

Capital. Anthropic's prospectus, as reported by Reuters, shows roughly $518 billion in total compute obligations (company-disclosed) CITED Source C41; DeepSeek's reported round would be one of China's largest AI financings FLAG Source C48. When labs carry obligations on that scale, pricing moves fast, as Haiku 5.5 shows VERIFIED Source C39. Plan for model swaps; do not lock workflows to one price sheet.

Not found in the window. No new McKinsey, BCG, Deloitte, Accenture, PwC, IDC, IMF, OECD, ILO, WEF or Stanford HAI report on agents that we could verify. The IMF and World Bank Annual Meetings fall after this edition.

10. Framework: the AEGIS Reversibility Ladder

The ladder sorts agent actions by how hard they are to undo, and assigns minimum controls to each rung. It is a practitioner model, not a standard PROPRIETARY Source C04. It plugs into AEGIS Pillar 2 (inventory: record each agent's rung), Pillar 4 (controls: match gates to the rung) and Pillar 6 (monitoring: test the stop path).

14. How-to: a seven-day kill-switch drill

  1. Day 1, pick one agent. Choose the agent with the highest rung, not the most users. Write its allowed actions on one page.
  2. Day 2, name the stopper. One primary and one backup person, with the access to halt it, in writing.
  3. Day 3, write the fallback. What the team does by hand when the agent is off. If nobody knows, the halt is not safe to pull.
  4. Day 4, check the log. Pull ten recent actions. Can you see inputs, tool calls, outputs and the approver? Is the log in your store?
  5. Day 5, run the halt. In a test window, halt the agent. Time it. Confirm the fallback runs.
  6. Day 6, reverse one action. Undo a test action end to end. Note anything that cannot be undone and who owns the remedy.
  7. Day 7, file the evidence. One page: rung, owner, halt time, undo result, gaps, dates. That page is what an examiner, auditor or board will ask for. File it under AEGIS Pillar 6.

The safest agents this month are the boring ones. In all four cases we studied, the deployed agent prepares, advises or loads a test rig. None of them decides. That is not a failure of ambition; it is how regulated firms get to production. The firms that will run Rung 3 and 4 agents in 2027 are the ones writing halt drills for Rung 1 agents now.

Is the CSBS framework a rule?

Do federal bank model-risk rules cover agents?

American Banker reports the April 2026 federal model-risk guidance left generative and agentic AI out; check the agency text before relying on that CITED Source C02. Until a separate federal process concludes, firms govern agents with their own controls.

What is a "kill switch" in practice?

A tested, documented way for a named person to stop an agent's actions within minutes, with a manual fallback so the business keeps running. A switch nobody has pulled is a hypothesis, not a control.
