Regulators just named who is allowed to unplug an AI agent. Enterprise software shipped the opposite the same week. Three financial regulators converged on one requirement within about three weeks. The FCA called agentic AI a "profound step change" on June 24. The Bank of England's Sarah Breeden said governance frameworks may need to change. FINRA's 2026 Annual Report reclassified agentic AI as an active supervisory priority. The Fed, OCC, and FDIC confirmed in SR 26-2 and Bulletin 2026-13 that existing model risk rules do not cover agentic systems. Strip away the different letterheads and it is the same ask: a named executive with documented authority to shut the system down, decided before it fails, not written up after. The same week, Salesforce announced AIforce at Dreamforce (Sep 15). Agents can now read across hundreds of records and act on customer data from Slack or Claude, without a person opening the Salesforce interface at all. Not a knock on the product. A timing observation: capability is scaling on one calendar, accountability infrastructure on another. Healthcare is running the identical pattern. KFF's August 13 analysis found 67 percent of Medicare Advantage prior authorization denials get overturned on appeal, but denials are rarely appealed. HHS-OIG found the overturn rate hits 95 to 97 percent for skilled nursing facility denials specifically. Neither report says whether the initial denials were agent assisted. That is the real finding: not that AI is deciding, but that nobody has published which decisions were. Financial services, healthcare, and federal procurement are converging on the same underlying question from three directions: who is accountable for what the agent did, and can you prove it before someone asks. An agent that acts faster than your governance can document is not a productivity story yet. It is a liability with good UX. Name the owner. Document the shutdown authority. Do it before the agent needs it.
